[Step 2] Obtain a contest form at Shell or print out the form.
Download Contest Form (make sure the print-out forms are stamped at the respective Shell stations).
[Step 3] Fill up the form and attach together your original receipt (pre-paid sales receipts are not accepted for this contest).
